WEST CARROLLTON, Ohio – Loose spark plugs on an airplane making its first flight three days after an annual inspection were the probable cause of a private airplane crash that took the life of a commercial airline pilot on June 17, 2008, the National Transportation Safety Board has reported.

WRIGHT-PATTERSON AIR FORCE BASE, Ohio — A safety system expected to save pilots’ lives and costly fighter jets is nearing production with the help of a senior Air Force Research Laboratory engineer here who shepherded its development for more than 20 years and put off retirement to see it through.
